At Foundations San Francisco, we believe treatment should meet you where you are, both personally and clinically. Whether you’re navigating anxiety, depression, workplace stress, substance use or a combination of mental health and addiction concerns, our goal is to help you regain stability and move forward with confidence.

Located in San Francisco’s Embarcadero District with views of the Bay, Foundations San Francisco provides evidence-based outpatient treatment for adults throughout San Francisco and Silicon Valley. Our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) offer flexible care that allows patients to receive comprehensive treatment while continuing to live at home or in supportive housing.

Many of the individuals we serve are balancing demanding careers while managing mental health conditions, substance use or both. Our integrated approach is designed to help patients improve their mental health, strengthen recovery and return to work and everyday life with the tools needed for long-term success.

Whether you’re beginning treatment for the first time or stepping down from residential care, our experienced team provides compassionate guidance every step of the way. To learn more about our programs, call 415.854.6735.

What Makes Foundations San Francisco Different?

Recovery isn’t one-size-fits-all, and neither is treatment.

At Foundations San Francisco, we take the time to understand your unique experiences, goals and challenges before developing your individualized treatment plan. Our experienced clinicians provide evidence-based care that addresses both symptoms and the underlying factors contributing to mental health and substance use disorders.

Our philosophy is built on several guiding principles:

  • Clinically sound, evidence-based treatment
  • Compassionate, individualized care
  • A welcoming and inclusive environment
  • Prompt assessments and responsive admissions
  • Flexible outpatient programming
  • Accountability, commitment and measurable progress
  • Integrated treatment for mental health and substance use disorders

We believe lasting recovery comes from treating the whole person, not simply managing symptoms.

When clinically appropriate, patients have access to psychiatric evaluation and ongoing medication management to help stabilize symptoms and support recovery. We take an integrated approach to your treatment, which may include individual therapy, group therapy, psychoeducation, family involvement, treatment planning and ongoing progress monitoring.

Our goal is simple: help every patient develop the skills, confidence and support needed to build a healthier future.

Our Treatment Programs

Foundations San Francisco offers a continuum of outpatient behavioral healthcare designed to provide the right level of support at every stage of recovery.

Partial Hospitalization Program (PHP)

Our Partial Hospitalization Program provides structured, intensive treatment for adults who need comprehensive clinical support while remaining connected to their daily lives. Patients participate in evidence-based therapies, psychiatric care and individualized treatment planning throughout the day before returning home or to supportive housing each evening.
